Insurance
We are in the process of credentialing with major insurance companies, both commercial and Medicare. In the meantime, we are implementing low-cost self-pay rates, that will always be available to patients without insurance or those that prefer to self-pay due to high-deductible plans. We are still able to offer itemized superbills for reimbursement direct from insurance companies to the patient, or for FSA/HSA reimbursement.
This does not effect your insurance coverage to order covered labs, medications, radiology, and other tests. This means that any labs we order will still be covered by your insurance plan, as well coverage for medications that may be prescribed and sent to your pharmacy, or outside testing.
We aim to keep our prices low and fair for self-pay visit rates, and offer constant discounts to supplements and vitamins available for patient purchase, and reduced cost of IV and injectable therapies to be as affordable as possible. We want your wellness and care to enhance, rather than hinder, your life and finances.
We also offer a multi-family member visit discount, and monthly patient membership VIP and for various services.

Costs are based on acuity & length of visit
15 minutes acute/sick visit - $37.50
20 minute follow-up - $50
30 minute follow-up - $75
60 minute follow-up- $150

The structure of this initial visit will consist of a 60 minute intake exam prior to labs, with an
included 30 minute follow-up with a lab-review, scheduled for 2 weeks after.
Established patient follow-up appointments (with labs) will have their labs ordered
and drawn at the lab, prior to their appointments, for expedited review.
Sport or Work Physical (no DOT) - $40
Initial ADHD/ASD screening (60 mins) - $175
FMLA/LOA/Disability Paperwork - $30
ESA Letter/Paperwork - $15
Family/Sibling Discount Visit - Based on Length of Time vs. Number of Patients Seen
For example, if a family has 3 children all exhibiting same symptoms of an illness, such as strep throat, if all 3 children are seen in the same 30 minute appointment - the visit will be $75 as opposed to a co-pay cost per patient.
VIP "Anytime" Membership:
Need a visit time that falls outside of normal business hours? Join our VIP membership program so you can reach your provider, 24/7, for telehealth visits. Christmas day and you have the flu? You will be able to call and have an appointment...It's a Sunday on vacation and you forgot your medication? Give a call and we will send it to the nearest pharmacy. At work late and need to have a visit at 7pm? You will have a telehealth visit at your convenience. This is a monthly membership that can be cancelled at anytime (will be active through duration of the month that was pre-paid), with no contract to have to commit to if you wish to discontinue this service for the future.
Individual: $500/month, Family: $850/month
Lab Testing ordered can be done at any Quest Diagnostics, Lab Corp, or Hospital Walk-in Lab with insurance.
Please let us know if you do not have insurance for lab coverage and we will work with Quest Diagnostics for a discounted self-pay rate.
Imaging will be referred out to Premier Radiology (X-ray, MRI, CT, U/S, Mammograms, ECHO, etc.) in the Nashville area. Premier radiology accepts most insurance plans and they also offer the best self-pay rates locally. If you are outside of the local service area, we can help research the best location for each patient to have these tests ordered and performed .
We currently do not offer sutures, joint injections, allergy shots, immunizations, suboxone management, Birth/GYN services, Physical Therapy, Chiropractic, Hyperbaric Oxygen Therapy, TMS, or Ozone treatments in order to be mindful of cost, time, and expertise - we are happy to give outside referrals for these services with local providers based on your individual needs.
Medication refills for maintenance medications (non-controlled substances) can be sent to a pharmacy for up to 1 year from a visit, if refills are requested after 1 year from last visit, patients must be seen in order to continue prescribing.
Controlled Substance Policy: Medication prescription quantity or refills for controlled substances vary based on federal (DEA) and TN state laws. If you are prescribed a controlled substance, the most up to date legal information will be discussed with the provider during the patient visit, including necessary legal visit frequency to obtain prescriptions or prescription refills.
Our policy is to provide the safest care for you while also protecting our licensure and practice. All patients will be required to undergo a UDS* (urine drug screen) per legal regulations, which may be several times per year, depending on the level of a controlled substance prescribed. Again, this will adhere to federal and state guidelines and laws.
The TN CSMD identifies all controlled prescriptions filled at the pharmacy. When a patient is looked up on the databased and is indicated by the CSMD to be attempting to obtain prescriptions from multiple providers simultaneously (“Doctor Shopping”), or has positive results for illicit substances ie. cocaine, meth, MDMA, or Rx medication that is NOT prescribed to the patient - that patient may be subject to discharge from the practice or put on a probationary plan at the providers discretion.
*UDS that are performed in office are an additional $5 cost to the patient
